Complete Your Forms
Download the new patient intake form below and fill it out ahead of time, so your visit is spent on you rather than paperwork.
History and Exam
Dr. Snowball reviews your medical history, talks through your symptoms and examines how the painful area moves. X-rays, a CT or MRI scan, or nerve testing may be recommended to confirm a diagnosis.
Your Treatment Plan
You leave with a clear plan. It may combine medical care, such as injections or EPAT, with therapies like physical therapy, or acupuncture.

- A list of current medications and allergies
- Reports from any recent X-rays, CT or MRI scans
- Records from other doctors you have seen for this problem

Telehealth appointments are open to new and existing patients. You meet your provider over secure video, so you can get care, advice and prescriptions without a trip to the office.

How do I schedule a telehealth visit?
Request a virtual appointment online or call the office. You will receive a calendar invitation with a link to the digital registration forms. Complete the forms, then use the same link to join your visit at the scheduled time.
Is telehealth covered by insurance?
Coverage depends on your plan. Many insurers cover telehealth, and Medicaid covers it in most states, but it is best to check with your insurance provider before your visit.
When is an in-person visit the better choice?
Treatments such as injections and EPAT need to be done in the office. Telehealth is also not for emergencies. If you have a medical emergency, go to your nearest urgent care center or hospital emergency department.
